Dual Enrollment Student/Parent Agreement Form


 

Wolf Pack Plus Students and Parents:
The Dual Enrollment Program at Lourdes University, “Wolf Pack Plus”, provides high school students with advanced learning opportunities and the ability to earn college credit. Dual Enrollment is an option for students who meet the requirements to take classes with Lourdes University either online or in person.

Please carefully read the list of conditions below, ask for clarification if you have questions or concerns, and then please sign and date as requested.


Student / Parent Agreement:
Responsibilities of the Student/Parent:
  1. Wolf Pack Plus Eligibility:Lourdes University allows eligible 10th-12th grade students with a 3.0 GPA or higher to register in a maximum of 12 credit hours per semester.
  2. Course Registration: Students are responsible for completing the required forms and all other application and registration paperwork in order to enroll as a dual enrollment student at Lourdes University. Online applications and forms are automatically sent to the designated parties (High School Guidance Counselor and Admissions Counselor at Lourdes). If required forms are not submitted by deadline student will not be able to enroll and take classes.
  3. Financial Responsibility:  Upon enrollment in classes, the student or their school will be billed by credit hour. Contact your high school guidance counselor to confirm what the financial agreement is and who is responsible for payment. Students must drop or withdrawal from a course before the add/drop date in order to have the charges removed from the account.  Not attending class does not facilitate a drop or withdrawal.
  4. Book Fees:Many school districts cover the cost of tuition and books for courses offered at the school’s location. Check with your high school guidance counselor on your school’s policy.
  5. Courses: Dual enrollment classes contain standard college course content.  As a private institution we assume no responsibility for supervision of minor students. Course issues should be addressed by the student with the course instructor.  If the issue cannot be resolved with the instructor, the student should contact the high school guidance counselor.
  6. Student Grades: The grade(s) the student earns in their dual enrollment Wolf Pack Plus course(s) with be part of their permanent grade, official college academic record and standing. Please refer to the current college catalog and student handbook for more information.
  7. Policy and Procedure: All Dual Enrollment students at Lourdes University are required to follow the rules and regulations of the university as published in the Lourdes University CatalogStudents must follow class schedule, check their Lourdes email, and set up/use the Student Portal and Canvas course sites. If they experience issues, it is the student's responsibility to contact the appropriate person for assistance (ie. Lourdes IT HelpDesk, Lourdes academic advisor, professor, or high school guidance counselor).
  8. Changes in Enrollment: Students are responsible for their enrollment status.  Student’s needing to add, drop, or otherwise change their initial course schedule must communicate the changes to their high school guidance counselor who will facilitate the change with Lourdes University.

As the parent or legal guardian of the student listed above, your signature on this document certifies that you have:

  • Read, understand, and agree with the policies and requirements listed above
  • Reviewed and discussed your son's/daughter's ability to succeed in the classes recommended by the high school guidance counselor
  • Give your consent to have your son/daughter enrolled in Wolf Pack Plus courses at Lourdes University
